You may request Enrollment or Degree Verifications by choosing one of the following:
MyOTC: Login to MyOTC and click on Enrollment Verification Request on the Access OTC tab to submit your request.
National Student Clearinghouse: Click on Order-Track-Verify and choose Enrollment Verification. The National Student Clearinghouse will charge a fee for your request(s). Degree Verification cost is $9.95, Verification of Dates of Attendance cost is $9.95, and Verification of Current Enrollment cost is $2.50.
Degree or Enrollment Verification and/or Request to Release Information form: Print, complete and submit the request form to Student Services at:
*Requests for pick up will be available with a photo ID after 2:00 p.m. the following business day after the form is received.
